Pass one
Take direct questions with a known method. Set a stop rule: if representation or first operation is unclear after a short scan, mark and move.
Pass two
Return in order of expected value: high confidence and moderate time before low confidence and long time. Apply any applicable marking rule only from the current official notice.

Classify every loss as concept, representation, reading, calculation, verification, selection, or time. Write one prevention action. “Careless” is not actionable; “converted 2.5 hours to 150 minutes after using it in a ratio” is.
